What to Bring to Your Appointment
Please contact our office to schedule an appointment. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know our Podiatrists. We accept most insurances.
Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
Please arrive 30 minutes early for your appointment to complete any remaining paperwork.
Patients photo ID
Patient's Health insurance card
List of patient’s current prescriptions and/or over-the-counter medication, including dose and frequency
Your primary care doctor’s contact information and date last seen
A referral from your primary care doctor (if required by your insurance carrier)
Any previous x-rays, MRI’s other test results relating to any past foot health conditions
If you are a diabetic – It would be helpful to either bring or have your primary care physician fax me a copy of your most recent blood work.
Other Office Recommendation & Policies:
Remove all nail polish prior to your visit
Do not put any cream or ointments on your feet the day of your visit.
All minors (under 18) must be accompanied by an adult with authority to consent to the minor’s treatment.

What to Expect at Your Podiatry Appointment

01 CHECK-IN
Please arrive a few minutes before your appointment so you have time to check in and complete any necessary paperwork. Bring your photo ID, insurance information, and a current list of medications. If you have recent X-rays, test results, or medical records related to your foot or ankle, please bring those with you as well.

02 MEET DR. TODD CINDRIC
Your visit begins with a conversation about your feet and ankles and any concerns you may have. Dr. Todd Cindric will take the time to listen to your symptoms, learn about your medical history, and understand how your foot or ankle problem is affecting you. We encourage you to ask questions and let us know about anything that has been troubling you.

03 FOOT & ANKLE EXAM
Dr. Todd Cindric will carefully examine your feet and ankles to better understand what may be causing your symptoms. Your evaluation may include checking your circulation, skin, nerves, strength, flexibility, balance, and the way your feet function when you stand or walk. Every examination is tailored to your individual needs.

04 PERSONALIZED CARE PLAN
Once your examination is complete, Dr. Todd Cindric will explain what he has found and discuss the best treatment options with you. Whenever possible, we focus on straightforward, conservative care and solutions that make sense for your health and daily routine. You will have time to ask questions and understand your options before deciding on the next step.
